Boarding Form

Thank you for choosing our Boarding Facilities!

Our facilities will allow your pet to be as comfortable as possible while you are away. Please feel free to contact us for a tour of our facilities at any time. Please do not assume your boarding arrangements are confirmed until we have contacted you to confirm the requested dates.

IMPORTANT: Boarding dates and arrangements are not confirmed until you have received notification. A staff member will contact you by phone or email.

    Please take note of size, breed, and temperament when requesting pets to be boarded together. Fights can occur between kennel mates, especially if pets are not used to being away from home. DAC is not responsible for injuries should this occur when the owner has requested pets board together. If there is evidence of aggression, pets will be separated and owners will be charged accordingly.

    •••vaccination Requirements: In order for my pet to board, proof of current vaccinations must be on record.

    Cats: Rabies, FVRCP, & leukemia for outdoor cats Dogs: Rabies, Distemper & bordatella (within the last 12 months)

    ***Boarders presented with fleas/ticks/worms will be treated upon arrival with the appropriate medication at the owner's expense***

    ***Emergency Care: Should your pet require medical attention while staying with us, our veterinarians will perform the necessary treatment to stabilize the health/safety/comfort of your pet. We will call the emergency contact listed below and discuss any further treatment. If you are unreachable, a decision will be made on appropriate course of treatment at the discresion of the veterinarian. Any costs are the owner's responsibility at check-out. Please check the appropriate box to approve or deny emergency care.

    ***By selecting "decline" you are releasing Deerfoot Animal Clinic from any liability should your pet's condition result in injury or death. I certify that I am the owner or authorized agent of the owner of this animal. I accept full financial responsibility for the pet & the services they receive. I understand that any animal left 10 days or more after the designated discharge date shall be considered abandoned & become the property of the clinic. I understand that the financial obligation will remain my responsibility no matter the outcome. Should it become necessary to collect through an outside agency, I agree to pay all costs including attorney's fees. Full payment is expected at pick-up.
  • For the safety of your pets and our staff, we do not allow drop-offs or pick-ups while the clinic is closed. We will only accept or discharge pets during normal clinic hours:

    Monday- Friday 7am-12:30pm and 1 :30pm-6pm Sunday (pick-up ONLY) 4pm-5:30pm

    NO Saturday drop-off or pick-up NO Holiday drop-off or pick-up
